This exercise will serve as an opportunity for the management and staff that make up your Hospital Incident Command Team to work through scenario information and injects dealing with the activities associated with a mass casualty / fatality incident involving the release of hazardous materials following a train derailment.

The Lethal Vapors – A Mass Fatality Tabletop Exercise is part of the national Caduceus Shield Exercise Initiative.

Exercise Scenario

Tuesday – 2:39AM

After a long summer area residents finally have a break from the hot weather. During the evening’s local news the meteorologist forecasts overnight weather to be cool an comfortable and states “This will be one of those evening that area residents can turn off their AC and open their windows”.

The conductor of a freight train traveling through the region notices a railroad switch the train is approaching is set to redirect the train to a spur track. Immediately the conductor applies the brakes but the train is unable to stop and crashes into a locomotive located on the spur track. As the locomotives collide many of the freight trains cars, including two tank cars caring chlorine, derail.

Within moments of the derailment a toxic cloud begin to spread throughout the neighborhoods surrounding the train track.

Participation Information
Mass Fatality Registration will open once the exercise is scheduled From a conference room or similar location via the ONX System as part of your Hospital’s Incident Command Team
* Everyone who will be participating in the exercise must register for the following reasons:
  1. Prior to the exercise important Read Ahead materials will be e-mailed to all registered participants. These materials are important to ensure your participation in the exercise is successful.
  2. At the end of the exercise only registered participants will be able to complete and submit their individual exercise evaluation (Hotwash).
  3. Only registered participants will be able to access and print their Certificate of Participation once they have submitted their individual exercise evaluation.
